Eligibility for the BRIDGE living program
BRIDGE was created as a way to provide help for you to continue living in the community in your own home. Participants must meet certain requirements to be eligible. Contact us to learn more about this opportunity.
Eligibility Criteria
Eligibility criteria
Enrollment in a BRIDGE living program is voluntary. To enroll, individuals must meet the following criteria:
- You are 55 years old or older.
- You are certified by the state in which you live as meeting the need for the nursing home level of care
- You are able to live safely in the community with the help of ICDAC or CCO services at the time your enrollment.
- You live in the servicing area: Polk, Scott, and Story County.
